Diabetes study: 'Mindful eating' equals traditional education in lowering weight and blood sugar

ScienceDaily (Nov. 8, 2012) ? Eating mindfully, or consuming food in response to physical cues of hunger and fullness, is just as effective as adhering to nutrition-based guidelines in reducing weight and blood sugar levels in adults with Type 2 diabetes, a new study suggests.

In a comparison study of the effectiveness of the two types of behavioral interventions, participants lost about the same amount of weight -- an average of between 3 1/2 and 6 pounds -- and lowered their long-term blood sugar levels significantly after three months.

One treatment group followed an established diabetes self-management education program, with a strong emphasis on nutrition information. The other group was trained in mindful meditation and a mindful approach to food selection and eating. Both interventions, involving weekly group meetings, also recommended physical activity.

"The more traditional education program includes general information about diabetes, but with more emphasis on nutrition and food choice: What are different types of carbohydrates and fats and how many am I supposed to have? What should I look for when I read a food label? What are healthy options when dining out? That was the traditional diabetes education program," said Carla Miller, associate professor of human nutrition at Ohio State University and lead author of the study.

"We compared it to an intervention where mindful meditation was applied specifically to eating and food choices. This intervention group did not receive specific nutrition goals. We said we want you to really tune into your body before you eat. Take a few minutes to assess how hungry you are and make conscious choices about how much you're eating. Stop eating when you're full.

"We studied two very different approaches, and we found they both worked. This means people with diabetes have choices when it comes to eating a healthy diet," Miller said.

The research is published in the November issue of the Journal of the Academy of Nutrition and Dietetics.

Participants were adults between age 35 and 65 years and had been diagnosed with Type 2 diabetes for at least one year. To be eligible, they had to have a body mass index, a measure of weight relative to height, of 27 or more, indicating they were overweight, and a hemoglobin A1c (HbA1c) reading of at least 7 percent. HbA1c measures blood glucose levels in the previous two to three months; normal HbA1c is 5.6 percent or lower.

Study participants were randomly assigned to a treatment group. Twenty-seven completed the mindful eating program, and 25 completed the traditional diabetes self-management program called "Smart Choices." Each intervention involved eight weekly and two biweekly 2 ?-hour sessions with trained facilitators.

Trainers of the mindfulness program encouraged participants to cultivate "inner wisdom," or mindful awareness related to eating, and "outer wisdom," which referred to personal knowledge of optimal nutrition choices for people with diabetes. Each session included guided meditation oriented toward participants' experiences and emotions associated with food. Participants received CDs for help with home meditation practice.

"We have so many environmental cues to eat in America that we've tuned out our normal physiological signals to eat. Being mindful means stopping long enough to become aware of these physiological cues," Miller said. "We also tried to generate awareness, staying in the moment, and living and eating in response to hunger instead of habits and unconscious eating."

The mindful intervention also included basic information about what is known as medical nutrition therapy: the relationships among calories consumed, carbohydrate and fat intake, weight regulation and high blood sugar.

In contrast, the Smart Choices program focused specifically on the condition of diabetes itself, including factors that can lead to the diagnosis, common complications (which include heart disease, kidney and nerve damage, eye problems and stroke), the importance of blood sugar control, and appropriate food choices when blood sugar levels spike. Every session included a medical nutrition therapy discussion such as calorie-intake goals, percentages of carbohydrates and fats in an ideal diet, and portion control. Many sessions included a 15- to 20-minute walk to further emphasize the recommendation for regular physical activity. Problem-solving regarding choosing healthy foods in high-risk situations, such as the holidays, were a focus of the program.

The interventions took place over three months. Researchers assessed participants' health measures and dietary habits immediately after the programs concluded and then again three months later at the study's end.

Weight loss and improvements in HbA1c levels were similar for both groups at the six-month follow-up point. Smart Choice participants as a group lost more weight, an average of 6 pounds, than did the mindful eating group, which lost an average of 3.5 pounds -- but the difference was not significant when analyzed statistically, Miller said.

HbA1c levels dropped in both groups as well, by between about 0.7 and 0.8 percent. "That was a clinically meaningful reduction in Hba1c, equivalent to what you would get on some diabetes medications," Miller said. "If the reduction were sustained over time, it would mean a dramatic reduction in complications associated with diabetes."

Both groups also similarly reduced calorie intake and lowered their consumption of foods with a high glycemic index, which are digested quickly and can drive up blood sugar.

Miller said that because nutrition education is particularly important to people with a new diabetes diagnosis, she sees the mindful meditation and eating option as a potential supplement to basic diabetes education that patients need.

She also said that participants adapted well to the concept of mindfulness even though it is generally considered an alternative health practice.

"One of the things we were evaluating was how well this was accepted by people who had no experience with it. It was very well accepted by participants in that group," she said. "And this tells us that people with diabetes have choices.

"The fact that both interventions were equally effective suggests that we should let people choose. If mindful meditation is appealing and people think that approach is effective, then it very well could be the best choice for them."

The Baby Boomer population is defined as the children born after WWII from the years 1946 to 1964. As the Baby Boomers get older many they are trying to retain their youthful looks more than ever. Older men and women are actually helping to increase the number of cosmetic surgeries performed every year. The population of the US is getting older but unlike previous generations, this population has placed a much more serious value on physical beauty.

In fact, some reports have shown that more attractive people actually make more money than their less attractive co-workers. Lots of people are saying in the work force longer than they used to and those more mature people want to look as good as their more youthful office mates and co-workers. There are many of different factors behind the increase in the popularity of plastic surgery. Reality plastic surgeon television shows compete with dramatic shows featuring good-looking plastic surgeons helping all sorts of equally sexy people. Cosmetic surgery is more accepted now.

People from this older generation have a healthy eating regimen, they exercise regularly and they want to look as youthful as they still feel. They are the first group of people to grow up with a real emphasis on healthy living. Cosmetic surgery can make a person?s external body look as good as they may feel on the inside. While some young people turn to cosmetic surgery to attract a love interest some older people use cosmetic surgery to simply look younger by a few years and improve their self-esteem.

The most popular cosmetic surgery request by far is the injection of Botox to smooth out wrinkly skin. Many of the more frequently performed procedures are meant to improve the skin, especially after decades of growing up without sun block. Skin and surface procedures such as microlaser facial peels, Botox injections, and wrinkle filling gels are often the first cosmetic surgical procedures tried.

Men usually opt for cosmetic procedures such as eyelid tucks, face tightening and other operations that make them look more youthful at the office. It is not only women who are visiting the local cosmetic surgeon. Most people think of plastic surgery as something that is more attractive to women, but that?s not always the situation. It?s true that many older women are having so-called ?mommy make-overs? which usually is composed of breast augmentation, a breast raising and a tummy tuck.

The expanding Baby Boomer population is more youth oriented and more interested hanging on to their natural health for as long as possible. As more and more mature men and women continue to search for ways to look younger there will be more cosmetic surgeons there to serve them. Plastic surgery is not just for the young anymore ? it is now a valid way for Baby Boomers to feel good about themselves.

Is housework a health hazard? Sheet-fitting palsy described by physicians

ScienceDaily (Nov. 8, 2012) ? What do a tight, fitted bed sheet and a blood clot in the wrist have in common? Both are associated with a condition called sheet fitting palsy. True to its name, the palsy is reported in those who spend a long period of time repeatedly trying to pull a fitted bed sheet over the corner of a mattress. But it has also been reported in basketball players and in those who do push-ups as exercise.

The injury is caused by the continuous flexing movement of the wrist and results in a tiny stroke in the artery to the hand. The resulting clot cuts off blood flow to the median nerve and produces symptoms that include numbness or weakness.

A case study was described recently by Wake Forest Baptist Medical Center neurologist Francis O. Walker, M.D., and colleagues, and published in the September edition of the Journal of Clinical Neuromuscular Disease.

The case involved a fit and active 73-year-old woman, who was a bit miffed last fall when she had to depend on her granddaughter to help her with some housework. No matter how hard she tried, she couldn't slip the last corner of a fitted sheet over a mattress. Sometimes she noticed a 'pins and needles' feeling in her hand, but didn't think anything of it. After shaking her hand for a while, the numbness would go away.

A few weeks later, facing the fitted sheet task without her granddaughter to help her, the woman tried for some time to move the sheet over the mattress without success. When she moved her hand from the sheet her thumb dropped down, limp. That's when she called her doctor, Mary F. Lyles, M.D.

On examination, Lyles saw that her patient had lost most of the ability to perform simple tasks such as tying a shoelace or buttoning a sleeve with her right hand. Suspecting acute carpel tunnel syndrome, Lyles called on neurologist Walker to perform nerve conduction studies and image the hand using ultrasound. Few physicians have seen what Walker saw in the study. "High resolution color flow Doppler showed a rare persistent median artery that had blood flow in it in the forearm, but not at the wrist where it was next to a swollen, injured, median nerve." The patient is among the five percent of people who have persistent median artery, a vessel that usually disappears after birth.

Shortly after her tests, the patient underwent surgery to relieve some of the pressure on the nerve, but her limp thumb persisted. The woman's surgeon advised her to strengthen the thumb by going through the motions of performing her everyday activities. Even if a patient can't do them, trying the daily tasks can help. Little by little, the strategy worked. At first she was successful picking up a paper clip. When she could open and close a chip clip, she knew she was back to normal. It had taken nearly a year for the nerve in the wrist to grow the new pathways that allowed her thumb to function.

For those with persistent median artery, repetitive motion can mean trauma to the wrist and lead to severe symptoms of carpal tunnel syndrome. And something as simple as making a bed can be a hazard, especially if the fitted sheet does not fit.

The Ontarion - Movie Review: Stories We Tell

Tabloids, gossip and reality television indicate how much our society enjoys prying into other people?s lives. These many different manifestations of voyeurism vary in degree of credentials, as it seems we?ve grown exhausted of shows featured on Slice (formerly Life) and Much Music. For good reason, documentaries haven?t lost their credentials; a documentary is the art world?s way of capturing history, making observations and teaching others, often in an accessible fashion. Toronto has both its own documentary festival titled Hot Docs and a theatre dedicated to screening documentaries ? two great gems worthy of the one and a half hour trip into the big city from Guelph.

I spent a good chunk of the last six years living in Toronto, a set for many films that turns into a star-studded environment a few times each year with the coming of the TIFF and music festival, NXNE. While these attractions come and go, Toronto has its share of longstanding famous residents. For instance, both Rachel McAdams and Ryan Gosling were voted best local female and male actors respectively in Now magazine?s Best of Toronto issue. Auteur Sarah Polley, a finalist in the rat race, is madly in love with Toronto and is the true winner, in my opinion. Born and raised in the city, the thirty-three year-old got her started on the CBC program Road to Avonlea. Polley slowly built a higher-profile reputation in films like Go, Dawn of the Dead and Splice. Meanwhile, Polley has written and directed works such as Away From Her, Take This Waltz and most recently, the feature Stories We Tell. Amongst the buzz, Polley has always resided in Toronto. For the few precious years I resided in Toronto, I often saw Polley doing regular-people things around the city such as riding the streetcar or shopping at a Kensington Market grocer. With each time I crossed paths with this Canadian gem, my heart fluttered.

Polley?s latest Stories We Tell is an interesting take on a documentary-style film. I appreciate documentaries that do not coddle the audience by telling them too much; this documentary eloquently shares stories from many different people that Polley does not once acknowledge as her family, but you know from their appearance and comfortable humour that they are related.

The film?s pastiche nature weaves together a number of memories through home movies, interviews, and a very delicate story style account that was written by her father. This account was read throughout the entire film to serve as the narrative for the interviews Polley conducted and home movies she showed.

Countless reviews spoil the ending, but I won?t ruin your experience in my account here. I will, however, gush that Polley, beyond being a thoughtful and brilliant actor, director and writer, has created a simple feature that will make you think. While this documentary pries deep into the Polley family history in a revealing and invasive fashion, many intricacies of family, history and gender roles are touched upon.

The film is bookended by two lovely contemporary tunes that are dear to me. The first tune played in the film was ?Skinny Love? by Bon Iver, which caught me off guard as it wasn?t suggested in the film?s trailer. The tune that was used in the trailer and at the end of the film was the hauntingly eerie tune ?Demon Host? by Toronto artist Timber Timbre.
